We want to hear from you!  Please take the Community Health Needs Survey

TGH and the All4HealthFL Collaborative are working together to get the community’s perspective on the most pressing health and wellness needs. Our organizations are working together to get the community’s perspective on the most pressing health and wellness needs.  Information collected will be used to develop programs and services to address the identified issues and measure impact.    We would not be able to capture the perspectives of our diverse community without your participation.

Community feedback and data captured from prior needs assessments lead to several initiatives to address the needs faced by so many in our community.  This included programs such as mental health first aid, the launch of school-based food pantries and other initiatives addressing food insecurity, reducing barriers in access to care, children’s health and safety programming, and so much more.

Your voice is important and will help us create programs and offer services to meet the needs of our community.  This survey is open to people residing in Hillsborough, Polk, Pasco, Pinellas Citrus, Hardee, Hernando, Highlands, Manatee, Marion, and Sarasota Counties until February 14th, 2025. We would appreciate you taking 15 minutes to fill out the survey below.  Thank you!
